Фармакологические свойства
Оказывает противовоспалительное, анальгезирующее и жаропонижающее действие, связанное с подавлением циклооксигеназы 1 и 2, регулирующих синтез простагландинов. Уменьшает агрегацию, адгезивность тромбоцитов и тромбообразование за счет подавления синтеза тромбоксана А2 в тромбоцитах. Антиагрегационный эффект сохраняется в течение 7 суток после однократного приема (больше выражен у мужчин, чем у женщин).
Растворяясь в воде, таблетка Упсарина Упса образует буферный раствор, который после приема препарата поддерживает действующие вещества в растворенном виде, не позволяя им при контакте с кислой средой желудка переходить обратно в нерастворенную форму и осаждаться в виде твердых частиц на его стенках. В результате обеспечивается более быстрое и полное всасывание препарата, а также его лучшая переносимость по сравнению с обычными таблетками ацетилсалициловой кислоты.
Показания к применению
Умеренно или слабо выраженный болевой синдром у взрослых различного происхождения: головная боль (в том числе связанная с алкогольным абстинентным синдромом), зубная боль, мигрень, невралгия, грудной корешковый синдром, мышечные и суставные боли, боли при менструациях.
Повышенная температура тела при простудных и других инфекционно-воспалительных заболеваниях (у взрослых и детей старше 15 лет).
- повышенная чувствительность к ацетилсалициловой кислоте, другим НПВП и компонентам препарата;
- зрозивно-язвенные поражения (в фазе обострения) желудочно-кишечного тракта, желудочно-кишечное кровотечение;
- выраженные нарушения функции печени или почек;
- «аспириновая астма»;
- геморрагические диатезы (гемофилии, болезнь Виллебранда, телеангиоэктазии, гипопротромбинемия, тромбоцитопения, тромбоцитопеническая пурпура);
- расслаивающаяся аневризма аорты;
- портальная гипертензия;
- дефицит витамина К;
- дефицит глюкозо-6-фосфатдегидрогеназы, фенилкетонурия:
- беременность (I и III триместр), период грудного вскармливания.
Препарат не назначают детям до 15 лет с острыми респираторными заболеваниями, вызванными вирусными инфекциями, из-за риска развития синдрома Рейе (энцефалопатия и острая жировая дистрофия печени с острым развитием печеночной недостаточности).
С осторожностью – гиперурикемия, уратный нефролигиаз, подагра, язвенная болезнь желудка и/или двенадцатиперстной кишки (в анамнезе), декомпепсированная сердечная недостаточность.
Способ применения и дозы
Препарат предназначен для взрослых и детей старше 15 лет.
Внутрь, взрослым и детям старше 15 лет: принимать по 1 таблетке (500 мг) до 6 раз в сутки. При сильных болях и высокой температуре – по 2 таблетки (1 г); максимальная суточная доза не должна превышать 6 таблеток (3 г).
Пожилые люди: принимать по 1 таблетке (500 мг) до 4 раз в сутки; максимальная суточная доза не должна превышать 4 таблеток (2 г).
Способ применения: растворить таблетку/таблетки в стакане (200 мл) воды или сока.
Частота и время приема: интервал между приемами препарата должен составлять не менее 4 часов. Регулярное соблюдение режима приема препарата позволяет избежать резкого повышения температуры и уменьшить интенсивность болевого синдрома.
Длительность лечения (без консультации с врачом) не должна превышать 5 дней при назначении в качестве обезболивающего средства и более 3 дней – в качестве жаропонижающего средства.
В рекомендованных дозах препарат обычно хорошо переносится. В редких случаях могут возникать следующие побочные эффекты:
- аллергические реакции – кожная сыпь, бронхослазм, отек Квинке;
- формирование на основе гаптенового механизма «аспириновой» триады (сочетание бронхиальной астмы, рецидивирующего полипоза носа и околоносовых пазух, и непереносимости ацетилсалициловой кислоты и препаратов пиразолонового ряда);
- желудочно-кишечные расстройства – тошнота, рвота, боли в эпигастральной области, диарея (понос), желудочно-кишечные кровотечения (рвота типа «кофейной гущи», черный «дегтеобразный» стул); снижение аппетита, повышение активности «печеночных» трансаминаз;
- нарушение функции почек;
- тромбоцитопения, анемия, лейкопения, гипербилирубинемия;
- геморрагический синдром (носовое кровотечение, кровоточивость десен), увеличение времени свертываемости крови;
При появлении каких-либо побочных эффектов необходимо немедленно прекратить прием препарата и проинформировать лечащего врача.
Передозировка
В начальной стадии отравления развиваются симптомы возбуждения центральной нервной системы, головокружение, сильная головная боль, снижение остроты слуха, нарушение зрения, тошнота, рвота, усиление дыхания. Позже наступает угнетение сознания вплоть до комы, дыхательная недостаточность, нарушения водно-электролитного обмена.
Лечение: при признаках отравления следует вызвать рвоту или сделать промывание желудка, назначить активированный уголь и слабительное и обратиться к врачу. Лечение следует проводить в условиях специализированного отделения.
Взаимодействие с другими лекарственными средствами
Ацетилсалициловая кислота усиливает токсичность метотрексата, эффекты наркотических анальгетиков, других НПВП, пероральных гипогликемических препаратов, гепарина, непрямых антикоагулянтов, тромболитиков и ингибиторов агрегации тромбоцитов, сульфаниламидов (в т.ч. ко-тримоксазола), трийодтиронина; снижает урикозурических препаратов (бензбромарон, сульфинпиразон), гипотензивных средств и диуретиков (спиронолактон, фуросемид).
Глюкокортикостероиды, алкоголь и алкогольсодержащие препараты увеличивают повреждающее действие на слизистую оболочку желудочно-кишечного тракта, повышают риск развития желудочно-кишечных кровотечений.
Ацетилсалициловая кислота повышает концентрацию дигоксина, барбитуратов и препаратов лития в плазме крови.
Антациды, содержащие магния и/или алюминия гидроксид, замедляют и ухудшают всасывание ацетилсалициловой кислоты.
Особые указания
Ацетилсалициловая кислота уменьшает выведение мочевой кислоты из организма, что может стать причиной острого приступа подагры у предрасположенных пациентов. При длительном применении препарата следует периодически проводить общий анализ крови и анализ кала на скрытую кровь, контролировать функциональное состояние печени. Перед хирургическим вмешательством, для уменьшения кровоточивости в ходе операции и в постоперационном периоде, следует отменить прием препарата за 5-7 дней и поставить в известность врача.
Детям нельзя назначать препарата, содержащие ацетилсалициловую кислоту, поскольку в случае вирусной инфекции повышается риск возникновения синдрома Рейе. Симптомами синдрома Рейе являются продолжительная рвота, острая энцефалопатия, увеличение печени. При необходимости применения препарата Б период лактации грудное вскармливание следует прекратить.
Больным, соблюдающим бессолевую или низкосолевую диету, следует учитывать, что каждая таблетка препарата содержит 388,5 мг (16,9 мэкв) натрия.

Таблетки шипучие 0,5 г. По 4 таблетки в стрипе из алюминиевой фольги, покрытой с внутренней стороны полиэтиленом.

Упсарин Упса – это нестероидный противовоспалительный препарат, оказывающий противовоспалительное, обезбаливающее и жаропонижающее действие.
Уменьшает агрегацию, адгезивность тромбоцитов и тромбообразование за счет подавления синтеза тромбоксана А2 в тромбоцитах. Антиагрегационный эффект сохраняется в течение 7 суток после однократного приема (больше выражен у мужчин, чем у женщин).
Снижает летальность и риск развития инфаркта миокарда при нестабильной стенокардии. Эффективен при первичной профилактике заболеваний ССС, особенно инфаркта миокарда у мужчин старше 40 лет, и при вторичной профилактике инфаркта миокарда.
В суточной дозе 6 г и более подавляет синтез протромбина в печени и увеличивает протромбиновое время.
Упсарин УПСА способен снижать уровень витамин К-зависимых свертывающих факторов (2,7,9,10), повышает показатель фибринолитической активности плазмы. На фоне лечения медикаментом значительно возрастает риск кровотечений и геморрагических осложнений при проведении оперативных вмешательств.
За счет нарушения реабсорбции мочевой кислоты в канальцах почечной системы ацетилсалициловая кислота стимулирует ее выведение.
При взаимодействии с водой таблетки Упсарин Упса растворяются и образуют буферный раствор, который способен поддерживать входящие в состав препарата вещества в растворенном виде даже при контакте с кислой средой желудка, благодаря чему обеспечивается полное и быстрое всасывание и лучшая переносимость.
Показания к применению
От чего помогает Упсарин Упса? Согласно инструкции, препарат назначают в составе комплексной терапии при болевом синдроме различного происхождения:
- головная боль, в том числе вызванная алкогольным абстинентным синдромом;
- мигрень;
- зубная боль;
- невралгия;
- мышечная боль, вызванная растяжением, ушибом, воспалительными процессами;
- болезненные менструации у женщин;
- суставные боли.
Также эффективен в качестве жаропонижающего средства при простудных и инфекционных заболеваниях. Применение Упсарин Упса от похмелья способствует снижению головной боли и других симптомов похмельного синдрома, разжижает кровь. Нужно отметить, что применение совместно с алкоголем увеличивает риск кровотечения в ЖКТ.
Инструкция по применению Упсарин Упса, дозировки
Принимают внутрь после еды, растворив таблетку в 100–200 мл чистой воды.
Дозировки взрослым и детям старше 15 лет по инструкции:
- по 1 таблетке Упсарин Упса 500 мг на прием до 6 раз в сутки.
- при сильных болях и высокой температуре — по 2 таблетки (1 г) на прием до 3 раз в сутки.
Минимальный интервал между приемами должен составлять 4 ч. Максимальная суточная доза не должна превышать 3 г (6 табл.).

От похмелья Упсарин Упса принимают в разовой дозировке 1 таблетка 500мг, увеличение дозировки не эффективно, но может привести к появлению побочных эффектов.
Согласно инструкции по применению, длительность приема Упсарин Упса (без консультации с врачом) не должна превышать 5 дней при назначении в качестве обезболивающего средства и более 3 дней — в качестве жаропонижающего средства.
Побочные эффекты
Инструкция предупреждает о возможности развития следующих побочных эффектов при назначении Упсарин Упса:
- Со стороны пищеварительной системы – изжога, боли в эпигастральной зоне, тошнота, обострение хронических гастритов и язвы, рвота, кишечные кровотечения, развитие реактивного панкреатита, увеличение активности печеночных трансминаз;
- Со стороны нервной системы – головокружения, шум в ушах, снижение слуха;
- Со стороны крови – тромбоцитопения, анемия, снижение цветного показателя и уровня эритроцитов в крови;
- Аллергические реакции – сыпь на коже, мелкие кровоизлияния под кожу, отек квинке, бронхоспазм, развитие бронхиальной астмы, ринит, ринорея, в редких случаях развитие анафилактического шока.

Противопоказания
Противопоказано назначать Упсарин Упса в следующих случаях:
- детский возраст до 15 лет;
- гиперчувствительность к ацетилсалициловой кислоте, другим НПВП и компонентам препарата;
- дефицит глюкозо-6-фосфатдегидрогеназы, фенилкето- нурия (препарат содержит аспартам);
- бронхиальная астма, индуцированная приемом салици- латов и др. НПВП в анамнезе;
- беременность (I и III триместр), период грудного вскармливания;
- эрозивно-язвенные поражения желудочно-кишечного тракта (в фазе обострения), желудочно-кишечное крово¬течение;
- геморрагические диатезы (гемофилии, болезнь Виллебранда, телеангиоэктазии, гипопротромбинемия, тромбоцитопения, тромбоцитопеническая пурпура);
- выраженные нарушения функции печени или почек;
- дефицит витамина К;
- глюкозо-галактозная мальабсорбция;
- одновременный прием с метотрексатом в дозе 15 мг/неделю и более;
- одновременный прием пероральных антикоагулянтов и ацетилсалициловой кислоты в дозе, превышающей 3 г в день.
Передозировка
Проявляется центральной гипервентиляцией легких, кровотечениями, анурией, респираторным алкалозом, коллапсом, сонливостью, метаболическим ацидозом и спутанностью сознания.
На первоначальном этапе гипервентиляция легких ведет к дыхательному алкалозу: цианоз конечностей и тела, удушье, одышка, появление холодного, липкого пота. Далее в результате усиления интоксикации развивается дыхательный паралич и полное разобщение в окислительном фосфорилировании, что в конечном итоге ведет к респираторному ацидозу.
При отравлении Упсарином Упса рекомендуется провокация рвоты, назначение слабительных средств, энтеросорбентов, активированного угля. Обязателен контроль над показателями электролитного баланса. При необходимости вводят раствор лактата, цитрата или гидрокарбоната натрия.
Усилить выведение ацетилсалициловой кислоты можно посредством повышения резервной щелочности. Для ощелачивания мочи не рекомендуется применить ацетазоламид (возможно развитие ацидемии, усиление токсического влияния ацетилсалициловой кислоты).
У лиц пожилого возраста интенсивная инфузионная терапия может привести к отеку легких, что необходимо учитывать при лечении интоксикации.
Гемодиализ назначают при уровне медикамента более 100-130 мг%.
При развитии отека легких назначают проведение искусственной вентиляции легких специальной смесью, богатой кислородом.

Особые указания

Тщательное измельчение таблеток, прием медикамента после еды, использование кишечнорастворимых форм ацетилсалициловой кислоты, применение лекарственных средств, которые нейтрализуют повышенную кислотность желудочного сока, позволяют уменьшить раздражающее влияние Упсарин УПСА на слизистые стенки пищеварительного тракта.
Лекарственное средство может спровоцировать у предрасположенных лиц новый приступ подагры. Такое воздействие объясняется снижением количества выводимой мочевой кислоты при лечении медикаментом.
Ацетилсалициловая кислота выделяется с грудным молоком, повышая риск кровотечений у младенцев из-за функциональных изменений в тромбоцитарных клетках крови.
Тератогенное влияние медикамента проявляется гипертонией в «малом» круге кровообращения, гиперплазией сосудов легочной ткани, преждевременным закрытием просвета артериального протока.
Лекарственное взаимодействие
Препарат не рекомендуется назначать пациентам на фоне применения других НПВП, ацетилсалициловой кислоты, Метотрексата. Данное лекарственное взаимодействие увеличивает вероятность развития тяжелых побочных эффектов.

Препарат Упсарин УПСА проявляет анальгезирующее, противовоспалительное, жаропонижающее, антиагрегатную действие.
Ацетилсалициловая кислота благодаря необратимому ингибированию ферментуциклооксигеназы тормозит биосинтез простагландинов. В очаге воспаления уменьшает проницаемость капилляров, снижает активность гиалуронидазы, ограничивает энергетическое обеспечение воспалительного процесса путем торможения образования АТФ. Снижает возбуждение центров терморегуляции и болевой чувствительности, уменьшает альгогенное диюбрадикинину. Кроме того, ацетил салициловая кислота ингибирует агрегацию тромбоцитов путем блокирования тромбоцитами синтеза тромбоксана А2.
Препарат выпускается в форме шипучих таблеток с буферными свойствами, уменьшает раздражающее действие ацетилсалициловой кислоты на слизистую оболочку желудочно-кишечного тракта.
Фармакокинетика
Шипучие растворимые таблетки препарата всасываются быстрее, чем обычные таблетки. Максимальная концентрация ацетилсалициловой кислоты в плазме крови достигается через 15-40 минут после приема. Биодоступность ацетилсалициловой кислоты изменяется в зависимости от дозы: она составляет примерно 60% при приеме менее 500 мг и 90% — при приеме более 1 гу связи с насыщением процесса гидролиза в печени. Ацетил салициловая кислота быстро гидролиза ется с образованием салициловой кислоты, которая также обладает фармакологической активностью. Ацетил салициловая кислота и салициловая кислота быстро распределяются по всем тканям организма. Эти кислоты проникают через плацентарный барьер, а также попадают в грудное молоко. Салициловая кислота интенсивно связывается с белками плазмы (90%). Период полувыведения из плазмы ацетилсалициловой кислоты составляет 15-20 минут, а салициловой кислоты- 2-4 часа.
Ацетилсалициловая кислота метаболизируется в печени и выводится в основном с мочой в виде салициловой, салицилуровои, ген тесовой кислот и глюкуронидов.
Показания к применению
Показаниями к применению препарата Упсарин УПСА являются:
— болевой синдром слабой и средней интенсивности при инфекционно-воспалительных заболеваниях (грипп, острые респираторные заболевания), а также невралгия, головная и зубная боль
— гипертермические реакции различного генеза;
— ревматологические заболевания;
— профилактика тромбозов, тромбоэмболии, инфаркта миокарда и ишемических нарушений мозгового кровообращения.
Способ применения
При интенсивной боли или гипертермии допускается прием сразу двух таблеток каждые 4:00, но не более 6 таблеток в сутки.
Лицам пожилого возраста суточная доза составляет 2 г ацетилсалициловой кислоты на сутки, то есть 4 таблетки, обычно назначают по одной таблетке 500 мг с интервалами не менее 4:00, но не более 4 таблеток в сутки.
Детям: с 9 лет с массой тела более 30 кг суточная доза не должна превышать 50 мг / кг массы тела. Препарат может назначаться детям с массой тела в менее 30 кг (обычно от 9 лет), с интервалом между приемами от 6 до 4 часов.
Пациент должен знать, что без консультации врача ацетил салициловукислоту можно принимать не более 3 дней для лечения гипертермической реакции тает более 5 дней для лечения болевого синдрома.
Побочные действия
Со стороны органов пищеварения: в некоторых случаях — тошнота, анорексия, боль в животе, эрозивно-язвенные изменения слизистой оболочки желудка и других отделов желудочно-кишечного тракта, желудочно-кишечное кровотечение.
Со стороны системы гемостаза: возможные геморрагический синдром (носовые кровотечения, кровотечения из десен, геморрагическая пурпура), удлинение времени кровотечения.
Реакции гиперчувствительности: крапивница, бронхоспазм, ангио невротичнийнабряк, анафилаксия.
Противопоказания
Противопоказаниями к применению препарата Упсарин УПСА являются:
— непереносимость ацетилсалициловой кислоты или других компонентов препарата
— фенилкетонурия, поскольку препарат содержит аспартам;
— бронхиальная астма, обусловленная приемом ацетилсалициловой кислоты или другими нестероидными противовоспалительными средствами;
— последние три месяца беременности;
— обострение язвенной болезни желудка и 12-перстной кишки;
— врожденный или приобретенный геморрагический диатез
— повышенный риск кровотечений;
— тяжелая печеночная недостаточность
— тяжелая почечная недостаточность
— тяжелая резистентная к лечению сердечная недостаточность
— сообщение с метотрексатом в дозе 15 мг / неделю;
— сочетание высоких доз препарата с непрямыми антикоагулянтами, особенно при лечении ревматических болезней;
— дети с массой тела менее 30 кг (младшие 9 лет).
Беременность
В период беременности (в I и II триместрах) возможен разовый прием препарата Упсарин УПСА.
Лактация. Ацетил салициловая кислота проникает в грудное молоко, поэтому применять препарат во время кормления грудью нецелесообразно.
Взаимодействие с другими лекарственными средствами
Комбинации, которые противопоказаны.
Пероральные антикоагулянты. При приеме ацетилсалициловой кислоты в дозах более 3 г / сут антикоагулянты вытесняются из соединений с белками плазмы.
Метотрексат в дозе 15 мг / неделю. Повышается Миелотоксические действие.
Нежелательные комбинации.
Пероральные антикоагулянты. Увеличение риска кровотечений, необходимо контролировать показатели свертывания крови, и особенно длительность кровотечения.
Другие нестероидные противовоспалительные препараты. Увеличивается риск возникновения язвы желудка и 12-перстной кишки и желудочно-кишечных кровотечений.
Гепарин. Увеличивается риск кровотечений (ингибирование функции тромбоцитов и повреждения слизистой оболочки желудочно-кишечного тракта).
Урикозурические средства (бензбромарон, пробенецид). Снижение урикозурического эффекта за счет конкуренции за выведение мочевой кислоты в почечных канальцах.
Комбинации, требующие осторожности.
Противодиабетические средства (инсулины). Посиленнягипогликемичнои действия.
Диуретики и ингибиторы фермента. При приеме в больших дозах возможно развитие острой почечной недостаности у больных с обезвоживанием (зниженняклуб клубочковой фильтрации через подавление синтеза простагландинов, кроме того снижается гипотензивное действие).
Глюкокортикоиды (при системном применении): снижение концентрации салицилатов в крови во время лечения кортикостероидами и риск передозировки салицилатов после их отмены (кортикостероиды усиливают выведение салицилатов).
Метотрексат в дозе менее 15 мг / неделю. Опасность миелотоксичности.

Внутренне маточные средства. Риск снижения контрацептивного эффекта.
Желудочно-кишечные средства местного действия оксидита гидроксиды магния, алюминия, соли кальция. Повышенное выведение салицилатов почками за счет лужнення мочи.
Передозировка
