Первичная профилактика острого инфаркта миокарда при наличии факторов риска: 50–100 мг/сут.
Профилактика повторного инфаркта миокарда, стенокардия: 50–100 мг/сут.
Профилактика инсульта и преходящего нарушения мозгового кровообращения: 50–100 мг/сут.
Профилактика тромбоэмболии после операций и инвазивных вмешательств на сосудах: 50–100 мг/сут.
Профилактика ТГВ и ТЭЛА : 100–200 мг/сут (2 табл.).

Показания к применению
Первичная профилактика острого инфаркта миокарда при наличии факторов риска (таких как сахарный диабет, гиперлипидемия, артериальная гипертензия, ожирение, курение, пожилой возраст);
Стабильная и нестабильная стенокардия;
Профилактика инсульта (в том числе у пациентов с преходящим нарушением мозгового кровообращения);

Противопоказания
Повышенная чувствительность к АСК, вспомогательным веществам в составе препарата и другим НПВП;
эрозивно-язвенные поражения желудочно-кишечного тракта (в фазе обострения);
бронхиальная астма, индуцированная приемом салицилатов и других НПВП;
сочетание бронхиальной астмы, рецидивирующего полипоза носа и околоносовых пазух и непереносимости АСК;
сочетанное применение с метотрексатом в дозе 15 мг в неделю и более;
беременность (I и III триместр) и период лактации;
возраст до 18 лет;
выраженная почечная недостаточность (клиренс креатинина (КК) менее 30 мл/мин);
выраженная печеночная недостаточность (класс В и выше по шкале Чайлд-Пью);
хроническая сердечная недостаточность III-IV функционального класса по классификации NYHA;
непереносимость лактозы, дефицит лактазы и глюкозо-галактозная мальабсорбция.
С осторожностью: при подагре, гиперурикемии, язвенной болезни желудка и 12-перстной кишки или желудочно-кишечных кровотечениях (в анамнезе), почечной недостаточности (КК более 30 мл/мин), печеночной недостаточности (ниже класса В по шкале Чайлд-Пью), бронхиальной астме, хронических заболеваниях органов дыхания, сенной лихорадке, полипозе носа, лекарственной аллергии, в том числе к препаратам группы НПВП, анальгетикам, противовоспалительным, противоревматическим средствам; беременности (II триместр), при предполагаемом хирургическом вмешательстве (включая незначительные, например, экстракция зуба).
С осторожностью при одновременном приеме со следующими лекарственными средствами:
— метотрексат (в дозе менее 15 мг в неделю);
— антикоагулянты, тромболитические или антитромбоцитарные средства;
— НПВП и производные салициловой кислоты в больших дозах;
— гипогликемические средства для приема внутрь (производные сульфонилмочевины) и инсулин;
— этиловый спирт (алкогольные напитки в частности);
— селективные ингибиторы обратного захвата серотонина;

Профилактика повторного инфаркта миокарда, стенокардия: 100 мг в сутки.
Профилактика инсульта и преходящего нарушения мозгового кровообращения: 50-100 мг в сутки.
Профилактика тромбоэмболии после операций и инвазивных вмешательств на сосудах: 50-100 мг в сутки.
Профилактика тромбоза глубоких вен и тромбоэмболии легочной артерии и ее ветвей: 100-200 мг (2 таблетки) в сутки.
Фармакологическое действие
Ацетилсалициловая кислота представляет собой сложный эфир салициловой кислоты. Необратимо инактивирует фермент ЦОГ-1, в результате этого блокируется синтез простагландинов, простациклинов и тромбоксана. Уменьшает агрегацию, адгезию тромбоцитов и тромбообразование за счет подавления синтеза тромбоксана А2 в тромбоцитах.
Повышает фибринолитическую активность плазмы крови и снижает концентрацию витамин К-зависимых факторов свертывания (II, VII, IX, X). Антиагрегантный эффект наиболее выражен в тромбоцитах, так как они неспособны повторно синтезировать циклооксигеназу. Антиагрегантный эффект развивается после применения препарата в малых дозах и сохраняется в течение 7 суток после однократного приема.
Оказывает также противовоспалительное, жаропонижающее и анальгезирующее действие.
Побочные действия
Побочные реакции при применении препарата наблюдаются в редких случаях.
Со стороны пищеварительной системы возможны тошнота, изжога, рвота, болевые ощущения в области живота; редко — язвы желудка и двенадцатиперстной кишки, в том числе перфоративные, желудочно-кишечные кровотечения, преходящие нарушения функции печени с повышением активности печеночных трансаминаз.
Со стороны ЦНС: головокружение, снижение слуха, шум в ушах, что может быть признаком передозировки препарата.
Со стороны системы кроветворения: повышенная частота периоперационных (интра- и постоперационных) кровотечений, гематом, носовых кровотечений, кровоточивости десен, кровотечений из мочеполовых путей. Имеются сообщения о серьезных случаях кровотечений, к которым относятся желудочно-кишечные кровотечения и кровоизлияния в мозг (особенно у пациентов с артериальной гипертензией, не достигших целевых показателей артериального давления и/или получавших сопутствующую терапию антикоагулянтными средствами, которые в отдельных случаях могут носить угрожающий жизни характер. Кровотечения могут приводить к развитию острой или хронической постгеморрагической/железодефицитной анемии (например, вследствие скрытого кровотечения) с соответствующими клинико-лабораторными симптомами (астения, бледность, гипоперфузия).
Аллергические реакции: кожная сыпь, зуд, крапивница, отек Квинке, ринит, отек слизистой оболочки полости носа, ринит, бронхоспазм, кардио-респираторный дистресс-синдром, а также тяжелые реакции, включая анафилактический шок.
Лёгкой и средней степени тяжести (однократная доза менее 150 мг/кг) возникают: головокружение, шум в ушах, снижение слуха, повышенное потоотделение, тошнота и рвота, головная боль, спутанность сознания; тахипноэ, гипервентиляция, респираторный алкалоз.
Лечение: желудочный лаваж, многократный прием активированного угля, форсированный щелочной диурез, восстановление водно-электролитного баланса и кислотно-щелочного состояния.
Средней и тяжелой степени тяжести (однократная доза 150 мг/кг-300 мг/кг — средняя степень тяжести, более 300 мг/кг — тяжелая степень отравления): респираторный алкалоз с компенсаторным метаболическим ацидозом, гиперпирексия, гипервентиляция,, некардиогенный отек легких, угнетение дыхания, асфиксия; нарушения ритма сердца, выраженное снижение артериального давления, угнетение сердечной деятельности; дегидратация, нарушение функции почек от олигурии вплоть до развития почечной недостаточности, характеризующееся гипокалиемией, гипернатриемией, гипонатриемией; нарушение метаболизма глюкозы:, гипергликемия, гипогликемия (особенно у детей), кетоацидоз; шум в ушах, глухота; желудочно-кишечные кровотечения; гематологические нарушения: от ингибирования агрегации тромбоцитов до коагулопатии, удлинение протромбинового времени, гипопротромбинемия; неврологические нарушения: токсическая энцефалопатия и угнетение функции центральной нервной системы (сонливость, спутанность сознания, кома, судороги).
В подобных случаях показана немедленная госпитализация в специализированные отделения для проведения экстренной терапии — желудочный лаваж, многократный прием активированного угля, форсированный щелочной диурез, гемодиализ, восстановление водно-электролитного баланса и кислотно-щелочного состояния, симптоматическая терапия.
Особые указания
Ацетилсалициловая кислота может провоцировать бронхоспазм, а также вызывать приступы бронхиальной астмы и другие реакции повышенной чувствительности (особенно при наличии бронхиальной астмы в анамнезе, сенной лихорадки, полипоза носа, хронических заболеваний дыхательной системы, а также аллергических реакций на другие препараты).
При приеме препарата возможно увеличение риска кровотечений в ходе оперативного вмешательства или в послеоперационном периоде. При необходимости абсолютного исключения кровоточивости в ходе оперативного вмешательства необходимо, по возможности, полностью отказаться от применения ацетилсалициловой кислоты в предоперационном периоде.
Ацетилсалициловая кислота в низких дозах может спровоцировать развитие подагры у предрасположенных лиц (имеющих сниженную экскрецию мочевой кислоты).
Ацетилсалициловая кислота в высоких дозах оказывает гипогликемический эффект, что необходимо иметь в виду при назначении препарата пациентам с сахарным диабетом, получающим сахароснижающие препараты и инсулин.
При совместном назначении глюкокортикостероидов и салицилатов следует помнить, что во время лечения уровень салицилатов в крови снижен, а после отмены ГКС возможна передозировка салицилатов.
Не рекомендуется сочетание ацетилсалициловой кислоты с ибупрофеном у пациентов с повышенным риском сердечно-сосудистых заболеваний, поскольку последний снижает кардиопротекторный эффект ацетилсалициловой кислоты.
При сочетании ацетилсалициловой кислоты с алкоголем повышается риск повреждения слизистой оболочки ЖКТ и удлинения времени кровотечения.
Препарат может вызывать головокружение, поэтому в период лечения необходимо соблюдать осторожность при вождении транспортных средств и занятиях потенциально опасными видами деятельности, требующими повышенной концентрации внимания и быстроты психомоторных реакций.
Применение при беременности и лактации
Применение при беременности
Применение больших доз салицилатов в первые 3 месяца беременности ассоциируется с повышенной частотой дефектов развития плода (расщепленное верхнее небо, пороки сердца). Применение салицилатов в I триместре беременности противопоказано.
В последнем триместре беременности салицилаты в высокой дозе (более 300 мг/сутки) вызывают торможение родовой деятельности, преждевременное закрытие артериального протока у плода, повышенную кровоточивость у матери и плода, а назначение непосредственно перед родами может вызвать внутричерепные кровоизлияния, особенно у недоношенных детей. Применение салицилатов в последнем триместре беременности противопоказано.
Во II триместре беременности салицилаты можно применять только с учетом строгой оценки риска и пользы для матери и плода, предпочтительно в дозах не выше 150 мг/сутки и непродолжительно.
Применение в период лактации
Салицилаты и их метаболиты в небольших количествах проникают в грудное молоко. Случайный прием салицилатов в период лактации не сопровождается развитием побочных реакций у ребенка и не требует прекращения грудного вскармливания. Однако, при длительном применении препарата или применении его в высокой дозе кормление грудью следует немедленно прекратить.
Взаимодействие
При одновременном применении ацетилсалициловая кислота усиливает действие метотрексата, дигоксина, гипогликемических средств для приема внутрь (производных сульфонилмочевины) и инсулина.
При одновременном применении с антикоагулянтами, тромболитическими и антиагрегантными средствами (тиклопидин, клопидогрел) отмечается увеличение риска кровотечений.
При одновременном применении с препаратами, обладающими антикоагулянтным, тромболитическим или антиагрегантным действием отмечается усиление повреждающего действия на слизистую оболочку ЖКТ.
При приеме селективных ингибиторов обратного захвата серотонина возможно повышение риска кровотечения из верхних отделов ЖКТ.
Увеличивает токсичность вальпроевой кислоты.
НПВС и производные салициловой кислоты в высоких дозах повышают риска ульцерогенного эффекта и кровотечения из ЖКТ. Ибупрофен снижает кардиопротекторные эффекты ацетилсалициловой кислоты.
Одновременное применение ацетилсалициловой кислоты в высоких дозах может уменьшать действие диуретиков, ингибиторов АПФ, препаратов с урикозурическим действием (бензбромарон, пробенецид).
При одновременном применении с системными глюкокортикостероидами (за исключением гидрокортизона, используемого для заместительной терапии болезни Аддисона) отмечается усиление выведения салицилатов и соответственно ослабление их действия.
При совместном применении с алкоголем (этанолом) повышен риск повреждения слизистой оболочки ЖКТ и удлинение времени кровотечения.
Условия хранения
Хранить в недоступном для детей, сухом и защищенном от света месте при температуре не выше 25°С.

Фармакологическое действие
Фармакодинамика
Ацетилсалициловая кислота (АСК) представляет собой сложный эфир салициловой кислоты, относится к группе нестероидных противовоспалительных препаратов (НПВП). Механизм действия основан на необратимой инактивации фермента циклооксигеназы (ЦОГ-1), в результате чего блокируется синтез простагландинов, простациклинов и тромбоксана. Уменьшает агрегацию, адгезию тромбоцитов и тромбообразование за счет подавления синтеза тромбоксана А2 в тромбоцитах.
Повышает фибринолитическую активность плазмы крови и снижает концентрацию витамин К-зависимых факторов свертывания (II, VII, IX, X). Антиагрегантный эффект наиболее выражен в тромбоцитах, так как они неспособны повторно синтезировать циклооксигеназу. Антиагрегантный эффект развивается после применения малых доз препарата и сохраняется в течение 7 суток после однократного приема. Эти свойства АСК используются в профилактике и лечении инфаркта миокарда, ишемической болезни сердца, осложнений варикозной болезни.
АСК оказывает также противовоспалительное, жаропонижающее и анальгезирующее действие.
Фармакокинетика
При приеме внутрь АСК всасывается быстро и полностью из желудочно-кишечного тракта. Таблетки Тромбо АСС покрыты кишечнорастворимой оболочкой, что уменьшает прямое раздражающее воздействие АСК на слизистую оболочку желудка. АСК частично метаболизируется во время абсорбции. Во время и после всасывания АСК превращается в главный метаболит — салициловую кислоту, которая метаболизируется, главным образом, в печени под влиянием ферментов печени с образованием таких метаболитов, как фенилсалицилат, глюкуронида салицилат и салицилуровая кислота, обнаруживаемых во многих тканях и в моче. У женщин процесс метаболизма проходит медленнее (меньшая активность ферментов в сыворотке крови).
АСК и салициловая кислота в высокой степени связываются с белками плазмы крови (от 66 до 98 %, в зависимости от дозы) и быстро распределяются в организме. Салициловая кислота проникает через плаценту и выделяется с грудным молоком.
Период полувыведения АСК из плазмы крови составляет около 15-20 минут. В отличие от других салицилатов, при многократном приеме препарата негидролизированная АСК не накапливается в сыворотке крови. Только 1% принятой внутрь АСК выводится почками в виде негидролизированной АСК, остальная часть выводится в виде салицилатов и их метаболитов. У пациентов с нормальной функцией почек 80-100% разовой дозы препарата выводится почками в течение 24-72 часов.
Показания к применению
— Первичная профилактика острого инфаркта миокарда при наличии факторов риска (например, сахарный диабет, гиперлипидемия, артериальная гипертензия, ожирение, курение, пожилой возраст);
— вторичная профилактика инфаркта миокарда (повторного);
— стабильная и нестабильная стенокардия;
— профилактика инсульта (в том числе, у пациентов с преходящим нарушением мозгового кровообращения);
— профилактика преходящего мозгового кровообращения;
— профилактика тромбоэмболии после операций и инвазивных вмешательств на сосудах (например, аорто-коронарное шунтирование, эндартерэктомия сонных артерий, ангиопластика и стентирование коронарных артерий);
— профилактика тромбоза глубоких вен и тромбоэмболии легочной артерии и ее ветвей (в том числе, при длительной иммобилизации в результате обширного хирургического вмешательства).
Противопоказания
— Повышенная чувствительность к АСК, вспомогательным веществам в составе препарата и другим НПВП;
— эрозивно-язвенные поражения желудочно-кишечного тракта (в фазе обострения);
— желудочно-кишечное кровотечение;
— геморрагический диатез;
— бронхиальная астма, индуцированная приемом салицилатов и других НПВП;
— сочетание бронхиальной астмы, рецидивирующего полипоза носа и околоносовых пазух и непереносимости АСК;
— сочетанное применение с метотрексатом в дозе 15 мг в неделю и более;
— беременность (I и III триместр) и период лактации;

Побочное действие
В целом Тромбо АСС ® вследствие низкой дозировки хорошо переносится больными, однако в редких случаях могут встречаться следующие нежелательные эффекты:
Со стороны пищеварительного тракта: тошнота, изжога, рвота, болевые ощущения в области живота; редко – язвы желудка и двенадцатиперстной кишки, в том числе перфоративные, желудочно-кишечные кровотечения, преходящие нарушения функции печени с повышением активности «печеночных» трансаминаз.
Со стороны центральной нервной системы: головокружение, снижение слуха, шум в ушах, что может быть признаком передозировки препарата (см. раздел Передозировка).
Со стороны системы кроветворения: повышенная частота периоперационных (интра- и постоперационных) кровотечений, гематом, носовых кровотечений, кровоточивости десен, кровотечений из мочеполовых путей. Имеются сообщения о серьезных случаях кровотечений, к которым относятся желудочно-кишечные кровотечения и кровоизлияния в мозг (особенно у пациентов с артериальной гипертензией, не достигших целевых показателей артериального давления (АД) и/или получавших сопутствующую терапию антикоагулянтными средствами), которые в отдельных случаях могут носить угрожающий жизни характер. Кровотечения могут приводить к развитию острой или хронической постгеморрагаческой/железодефицитной анемии (например, вследствие скрытого кровотечения) с соответствующими клинико-лабораторными симптомами (астения, бледность, гипоперфузия).
Аллергические реакции: кожная сыпь, зуд, крапивница, отек Квинке, ринит, отек слизистой оболочки полости носа, ринит, бронхоспазм, кардио-респираторный дистресс-синдром, а также тяжелые реакции, включая анафилактический шок.
Передозировка
Может иметь тяжёлые последствия, прежде всего, у пожилых пациентов и у детей. Синдром салицилизма развивается при приеме АСК в дозе более 100 мг/кг/сутки на протяжении более 2-х суток вследствие употребления токсических доз препарата в рамках неправильного терапевтического применения (хроническое отравление) или однократного случайного или намеренного приема токсической дозы препарата взрослым или ребенком (острое отравление).
Симптомы передозировки:

— средняя степень тяжести, более 300 мг/кг – тяжелая степень отравления): респираторный алкалоз с компенсаторным метаболическим ацидозом, гиперпирексия, гипервентиляция, некардиогенный отек легких, угнетение дыхания, асфиксия; со стороны сердечно-сосудистой системы: нарушения ритма сердца, выраженное снижение АД, угнетение сердечной деятельности; со стороны водно-электролитного баланса: дегидратация, нарушение функции почек от олигурии вплоть до развития почечной недостаточности, характеризующееся гипокалиемией, гипернатриемией, гипонатриемией; нарушение метаболизма глюкозы: гипергликемия, гипогликемия (особенно у детей), кетоацидоз; шум в ушах, глухота; желудочно-кишечные кровотечения; гематологические нарушения: от ингибирования агрегации тромбоцитов до коагулопатии, удлинение протромбинового времени, гипопротромбинемия; неврологические нарушения: токсическая энцефалопатия и угнетение функции центральной нервной системы (сонливость, спутанность сознания, кома, судороги).
Лечение: немедленная госпитализация в специализированные отделения для проведения экстренной терапии – желудочный лаваж, многократный прием активированного угля, форсированный щелочной диурез, гемодиализ, восстановление водно-электролитного баланса и кислотно-щелочного состояния, симптоматическая терапия.
Взаимодействие с другими лекарственными средствами
При одновременном применении АСК усиливает действие следующих лекарственных средств; при необходимости одновременного применения АСК с перечисленными средствами следует рассмотреть вопрос о необходимости уменьшения дозы указанных средств:
— метотрексата, за счет снижения почечного клиренса и вытеснения его из связи с белками;
— при одновременном применении с антикоагулянтами, тромболитическими и антиагрегантными средствами (тиклопидин, клопидогрел) отмечается увеличение риска кровотечений в результате синергизма основных терапевтических эффектов применяемых средств;
— при одновременном применении с препаратами, обладающими антикоагулянтным, тромболитическим или антиагрегантным действием, отмечается усиление повреждающего действия на слизистую оболочку ЖКТ;
— селективных ингибиторов обратного захвата серотонина, что может привести к повышению риска кровотечения из верхних отделов ЖКТ (синергизм с АСК);
— дигоксина, вследствие снижения его почечной экскреции, что может привести к передозировке;
— гипогликемических средств для приема внутрь (производных сульфонилмочевины) и инсулина за счет гипогликемических свойств самой АСК в высоких дозах и вытеснения производных сульфонилмочевины из связи с белками плазмы крови;
— при одновременном применении с вальпроевой кислотой увеличивается ее токсичность за счет вытеснения ее связи с белками плазмы крови;
— НПВП и производных салициловой кислоты в высоких дозах (повышение риска ульцерогенного эффекта и кровотечения из ЖКТ в результате синергизма действия); при одновременном применении с ибупрофеном отмечается антагонизм в отношении необратимого угнетения тромбоцитов, обусловленного действием АСК, что приводит к снижению кардиопротективных эффектов АСК;
— этанола (повышен риск повреждения слизистой оболочки ЖКТ и удлинение времени кровотечения в результате взаимного усиления эффектов АСК и этанола).
Одновременное применение АСК в высоких дозах может ослаблять действие перечисленных ниже лекарственных средств; при необходимости одновременного назначения АСК с перечисленными средствами следует рассмотреть вопрос о необходимости коррекции дозы перечисленных средств:
— любые диуретики (при совместном применении с АСК в высоких дозах отмечается снижение скорости клубочковой фильтрации (СКФ) в результате снижения синтеза простагландинов в почках);
— ингибиторы ангиотензинпревращающего фермента (АПФ) (отмечается дозозависимое снижение СКФ в результате ингибирования простагландинов, обладающих сосудорасширяющим действием, соответственно, ослабление гипотензивного действия. Клиническое снижение СКФ отмечается при суточной дозе АСК более 160 мг. Кроме того, отмечается снижение положительного кардиопротективного действия ингибиторов АПФ, назначенных пациентам для терапии хронической сердечной недостаточности. Этот эффект также проявляется при применении совместно с АСК в больших дозах).
— препараты с урикозурическим действием – бензбромарон, пробенецид (снижение урикозурического эффекта вследствие конкурентного подавления почечной канальцевой экскреции мочевой кислоты);
— при одновременном применении с системными глюкокортикостероидами (за исключением гидрокортизона, используемого для заместительной терапии болезни Аддисона) отмечается усиление выведения салицилатов и соответственно ослабление их действия.
Особые указания
Препарат следует применять после назначения врача.
АСК может провоцировать бронхоспазм, а также вызывать приступы бронхиальной астмы и другие реакции повышенной чувствительности. Факторами риска являются наличие бронхиальной астмы в анамнезе, сенной лихорадки, полипоза носа, хронических заболеваний дыхательной системы, а также аллергических реакций на другие препараты (например, кожные реакции, зуд, крапивница).
Ингибирующее действие АСК на агрегацию тромбоцитов сохраняется в течение нескольких дней после приема, в связи с чем, возможно увеличение риска кровотечений в ходе оперативного вмешательства или в послеоперационном периоде. При необходимости абсолютного исключения кровоточивости в ходе оперативного вмешательства необходимо, по возможности, полностью отказаться от применения АСК в предоперационном периоде.
Сочетание АСК с антикоагулянтами, тромболитиками и антиагрегантными препаратами сопровождается повышенным риском развития кровотечений.
АСК в низких дозах может спровоцировать развитие подагры у предрасположенных лиц (имеющих сниженную экскрецию мочевой кислоты).
Сочетание АСК с метотрексатом сопровождается повышенной частотой развития побочных эффектов со стороны органов кроветворения.
Высокие дозы АСК оказывают гипогликемический эффект, что необходимо иметь в виду при назначении ее пациентам с сахарным диабетом, получающим гипогликемические средства для приема внутрь (производные сульфонилмочевины) и инсулин.
При сочетанном назначении глюкокортикостероидов (ГКС) и салицилатов следует помнить, что во время лечения уровень салицилатов в крови снижен, а после отмены ГКС возможна передозировка салицилатов.
Не рекомендуется сочетание АСК с ибупрофеном у пациентов с повышенным риском сердечно-сосудистых заболеваний, поскольку последний снижает положительное влияние АСК на продолжительность жизни (снижает кардиопротективный эффект АСК). Превышение дозы АСК сопряжено с риском желудочно-кишечного кровотечения. Передозировка особенно опасна у пожилых пациентов.
При сочетании АСК с этанолом (алкогольные напитки) повышен риск повреждения слизистой оболочки ЖКТ и удлинения времени кровотечения.
