Троксевенол – это препарат для лечения хронической венозной недостаточности. Это комбинированное средство для местного применения.
Представлен в виде гомогенного прозрачного геля желтого цвета с легкой опалесцией.
Медикамент относится к капиляростабилизирующей группе средств.
Фармакологическое действие и фармакокинетика
Лекарство является производным рутина и имеет Р-витаминную активность. Снижает вероятность ломкости и проницаемости каппиляров.
Снимает отеки и участвует в окислительно-востановительных процессах организма. Сам препарат нетоксичен, обладает большим спектром терапевтического действия. При местном применении уменьшает болевой порог и укорачивает период функциональной реабилитации после повреждения опорно-двигательного аппарата.
Активные вещества препарата предотвращают появление сосудорасширяющего эффекта гистамина, брадикинина и ацетилхолина. По 
фармакологической группе медикамент относится к ангиопротекторам.
Медикамент имеет гелевую основу, которая обеспечивает полную растворимость активных компонентов и их проникновение в воспаленные участки. Содержащийся в составе препарата индометацин соединяется с белками плазмы крови более чем на 90% и проходит в печенке биотрансформацию. Индометацин выводится из организма вместе калом и мочой. Способен проникать в грудное молоко.
Гель способен быстро абсорбироваться с поверхности кожи, если применение наружное. Если прием внутренний, то средство хорошо выводится из желудочно-кишечного тракта. Терапевтическая концентрация медикамента в плазме крови поддерживается на протяжении восьми часов.
Форма выпуска и состав препарата
Лекарство выпускается в форме геля в алюминиевых тубах и банках. Емкость любого вида тары составляет 40г. Существует пять форм выпуска препарата:
- флакон из темного стекла (40г) в картонной упаковке;
- стеклянная банка (40г) в картонной упаковке;
- алюминиевая туба(40г);
- туба из комбинированных материалов (40г);
- ламинатная туба (40г).
В состав 100 г геля входит в 2 г активного вещества троксерутина, 3 г индометацина и вспомогательные составляющие в таком количестве:
- вода очищенная до 100 г;
- карбомер – 0,6 г;
- эздетат динатрия – 0,05 г;
- натрий гидроксид 30% — 0,7 г;
- хлорид бензалкония – 0,0575 г;
- этанол (96%).
Лекарственная форма препарата представлена в виде геля от желтого, до желто-зеленого цвета.
Механизм действия
Активное вещество медикамента начинает оказывать противоспалительное воздействие и снимает отечность.
При нанесении геля на пораженный участок, происходит снижение воспаления и ограничивается прилипание тромбоцитов к поверхности сосудистой стенки.
Действующее вещество лекарства блокирует гиалуронидазу и стабилизирует уровень концентрации гиалуроновой кислоты. Флавонид, который содержится в препарате, укрепляет плотность сосудистой стенки и предотвращает экссудацию жидкой части плазмы.
Содержащийся в препарате индометацин прекращает синтез простогландинов путем обратной блокады фермента циклогексана. Также действующие вещества способны предотвращать агрегацию тромбоцитов.
Показания и противопоказания к применению
Троксевенол применяется при наличии следующих заболеваний:
- варикозное расширение вен;
- повышенная проницаемость капилляров (геморрагический диатез);
- затяжная венозная недостаточность с сопровождением язвы голени и тяжести в ногах;
- трофическое поражение кожи и поверхностный тромбофлебит;
- наличие отеков после проведенных хирургических операций;
- ревматизм мягких тканей (бурсит, периартрит, фиброзит);
- растяжения, ушибы и вывихи;
- диабетическая микроангиопатия;
- проявление побочных сосудистых эффектов после лучевой терапии;
- комплексная терапия геморроя.
- язва желудка и двенадцатиперстной кишки;
- гиперчувствительность к медикаменту;
- наличие почечной недостаточности (при острой необходимости приема разрешается только кратковременное применение);
- длительный гастрит (в особенности фаза обострения);
- чувствительность к нестероидным противовоспалительным средствам;
- первый триместр беременности и период кормления грудью;
- возраст до восемнадцати лет;
- наличие бронхиальной астмы.
Схема приема и дозировки
Троксевенол в капсулах принимается во время еды, капсулы нужно проглатывать целиком. Кратность приема составляет три раза в день. В качестве поддерживающей терапии и в целях профилактики препарат назначается по одной капсуле два раза в сутки.
Дневная норма препарата составляет 70-90 мг. В среднем, продолжительность курса лечения составляет около трех-четырех недель. Назначение более длительного курса производиться исключительно в индивидуальном порядке.
Троксевенол в форме геля применяется наружно. На поврежденный участок наносится столбик геля высотой в 4 см и постепенно втирается в кожу. Суточная доза геля составляет 20 см. Курс лечения с применением геля не должен превышать более десяти дней. Кратность использования геля составляет 3-4 раза в день.
Передозировка и возможные «побочки»
Передозировка возникает при превышении суточной дозы и при длительном курсе лечения. В результате, может возникнуть сильная головной боль, гепатоксичность и кровоизлияние.
При внутреннем употреблении медикамента, возможно возникновение жжения в ротовой полости, тошнота, рвота и обильное 
слюноотделение. В таком случае, нужно провести промывание ротовой полости и желудка. При серьезном поражении этих органов, назначается профилактическое лечение.
Попадание препарата на слизистые оболочки и не затянутые раны вызывает раздражение, которое проявляется жжением, гипермией, слезотечением и болью.
Гель смывается с пораженного участка большим количеством воды или с помощью изотонического раствора натрия хлорида. Смывать гель следует до тех пор, пока он полностью не исчезнет или не уменьшится выраженность характерных симптомов.
К побочным действиям препарата относится возникновение аллергической реакции (кожная сыпь) и эрозивное поражение желудочно-кишечного тракта. Также возможно проявления симптома повышенной чувствительности (бронхиальная астма, анафилаксия).
Препарат не рекомендуется использовать лицам, которые не достигли четырнадцати лет. Это связано с отсутствием клинического опыта. Врачи не рекомендуют при прохождении курса лечения употреблять алкоголь, так как при этом увеличивается вероятность возникновения побочных действий.
Назначение медикамента запрещено на первом триместре беременности. При острой необходимости его можно применить только на втором и третьем триместре, но исключительно по назначению врача.
Взаимодействие с другими лекарствами
Троксевенол нельзя одновременно применять с другими стероидными противовоспалительными средствами. Также не рекомендуется параллельное применение с кортикостероидами, из-за возможности ульцерогенного действия.
При совместном применении с вышеуказанными препаратами, усиливается воздействие аскорбиновой кислоты на строение сосудистой стенки и ее проницаемость.

Для лечения варикоза и сопутствующих ему патологических состояний врачи рекомендуют препарат Троксевенол гель. Инструкция по применению этого наружного ЛС описывает его действие как ангиопротекторное и тонизирующее.
Имеет однородную структуру, цвет — прозрачно-желтый.
Действующее вещество и состав
В составе два активных компонента.
Индометацин — противовоспалительный и противоотечный компонент.
В 1 грамме содержится 0,02 г троксерутина и 0,03 г индометацина.
Другие компоненты: пропиленгликоль, этанол 96%, макрогол 400, карбомер 934, Е218.
Фармакодинамика и фармакокинетика
Фармакологическое действие Троксевенола обусловлено наличием двух активных веществ — индометацина и троксерутина.
Индометацин производит противовоспалительное и противоотечное действие, снимает боль. Действие вещества основано на его способности ингибировать синтез простагландинов путем обратной блокады фермента ЦОГ.
Препятствует залипанию тромбоцитов у стенок сосудов и склеиванию между собой. При наружном локальном использовании вещество снимает боль и отек, сокращает время восстановления двигательной способности при повреждениях и травмах костно-мышечной системы.
В сравнении с салицилатами и фенилбутазоном, имеет более сильные противовоспалительные свойства.
Троксерутин относится к биофлавоноидам, это смесь производных рутозида. Проявляет капилляропротекторное, антиоксидантное, сосудоукрепляющее действие. Снижает проницаемость капилляров и тонизирует вены.
Устраняет расширение сосудов, вызванное эффектом гистамина, брадикинина и ацетилхолина. Снимает отечность и улучшает трофику при болезненных проявлениях хронической венозной недостаточности.
Уменьшает экссудативное воспаление стенок сосудов и предотвращает прилипание к ним тромбоцитов.
Гелевая форма препарата позволяет активным компонентам проникать в воспаленные ткани и синовиальную жидкость. Биодоступность индометацина составляет 90%, метаболизм происходит в печени с образованием инертных соединений. Выводится с мочой, желчью и калом. Способен проникать в грудное молоко.
Троксерутин выводится почками.
Показания
Показан для комплексного лечения венозной недостаточности и варикоза, а также ревматических состояний и травм.
Эффективен при поверхностном тромбофлебите, флебите и после перенесенного флебита, геморрое, тендовагините, фиброзите, бурсите, периартрите.
В послеоперационном периоде назначают для снятия отеков и восстановления функциональной способности конечностей.
Средство помогает снять воспаление и устранить отечность после ушиба, вывиха, растяжения.
Противопоказания
Не назначают пациентам с бронхиальной астмой и склонностью к аллергическим реакциям на нестероидные противовоспалительные препараты.
Не применяют при склонностях к кровотечениям, геморрагическом дерматите, лейкопении, нарушении свертываемости крови.
Противопоказан Троксевенол детям до 14 лет, кормящим и находящимся в первом триместре беременности женщинам.
Способ применения и дозировка
Препарат назначает врач. Средняя продолжительность курса составляет 10 дней. Наносят 3-4 раза в день.
Максимальная суточная доза вещества составляет 20 сантиметров гелевого столбика. На один прием достаточно 4-5 см геля, выдавленного из тюбика.
Средство наносят на поврежденный участок, втирая легкими круговыми движениями.
В детском возрасте, при беременности и ГВ
Троксевенол не прошел достоверные клинические испытания, поэтому его не назначают детям до 14 лет и беременным в первом триместре беременности.
На более поздних сроках беременности и при грудном вскармливании ЛС используют с осторожностью, оценив, насколько ожидаемый эффект от лечения превышает возможные риски для малыша.
Побочные эффекты
Большинство людей хорошо переносят препарат и не сталкиваются с проявлением побочных эффектов.
Чаще всего пациенты встречаются с местными реакциями — покраснением кожи в месте нанесения геля, зуд и жжение, ощущение тепла и покалывания.
Системные реакции проявляются после очень долгого, неконтролируемого применения геля. Со стороны желудочно-кишечного тракта наблюдаются боль в желудке и эпигастральной области, тошнота и рвота, метеоризм. Повышаются печеночные трансаминазы.
Иммунная система может выдать отрицательный ответ в виде признаков повышенной сенсибилизации. Крайне редко отмечаются анафилактический шок, астматический приступ, ангионевротический отек.
В составе препарата содержится пропиленгликоль, который провоцирует раздражение кожи и Е218, способный стать причиной аллергии.
Особые указания
Пациентам, которым предписан курс лечения свыше 10 дней, проводят контроль для предупреждения и своевременного выявления побочных системных эффектов — токсичности, кровоизлияний.
Проводят лабораторные исследования крови на количество тромбоцитов и для контроля лейкоцитарной формулы.
Следует избегать занесения геля в конъюнктиву глаз, слизистые и активные раны. При занесении отмечается покраснение конъюнктивы и тканей, слезотечение, боль и жжение.
Пострадавшие участки смывают чистой водой или изотоническим раствором натрий хлорида до устранения неприятной симптоматики.
Случайное проглатывание может вызвать тошноту, рвоту, повышенное слюноотделение, боли в желудке. Промывают желудок и ротовую полость, проводят симптоматическое лечение.
Усиливает действие препаратов, вызывающих фотосенсибилизацию.
Передозировка
При правильном использовании ЛС передозировка невозможна. Если превышена длительность курса терапии и наблюдаются системные эффекты, отменяют применение и проводят симптоматическое лечение.

Имеет много аналогов с одним активным веществом:
Цены на эти препараты отличаются незначительно, и зависят от фирмы-производителя.
В продаже имеется полный аналог с комбинированным составом троксерутин+индометацин. Лекарственное средство называется Индовазин, его цена колеблется в пределах 270-350 рублей.
Такая же комбинация активных веществ присутствует и в препарате Троксиметацин, стоимость которого в аптеках составляет 150-200 рублей.

Троксевенол представляет собой флавоноид (производное рутина), обладающий Р-витаминной активностью; оказывает венотонизирующее, ангиопротекторное, противоотечное, антиоксидантное и противовоспалительное действие.
Троксерутин уменьшает проницаемость и ломкость капилляров, повышает их тонус; обладает антиоксидантной активностью. Предотвращает повреждение базальной мембраны эндотелиальных клеток при воздействии на нее различных факторов. Троксерутин снижает экссудативное воспаление в сосудистой стенке, ограничивая прилипание к ее поверхности тромбоцитов. В результате лечения исчезает чувство тяжести в ногах, уменьшается отечность тканей, улучшается трофика.
Побочные действия
В редких случаях наблюдаются аллергические реакции (покраснения кожных покровов, зуд, сыпь). Эти симптомы быстро проходят после прекращения лечения.
Нет данных о передозировке при наружном применении Троксевенола. При случайном проглатывании препарата возможны ощущение жжения в полости рта, слюнотечение, тошнота, рвота. В таком случае показано промывание полости рта и желудка, при необходимости — симптоматическое лечение.
При попадании в глаза, на слизистые оболочки и открытые раны наблюдается местное раздражение — слезотечение, гиперемия, жжение, боль. Гель смывают большим количеством дистиллированной воды или изотонического раствора натрия хлорида до полного исчезновения или значительного уменьшения выраженности указанных симптомов.
Особые указания
Необходимо избегать попадания препарата на открытые поверхности ран, слизистые оболочки, и в глаза.
