Гиполипидемические препараты (ATC код C10) — группа веществ, лекарственные препараты для снижения концентрации некоторых фракций липидов (в частности, т. н. ЛПНП) в тканях и жидкостях организма.

История [ править | править код ]
В начале XX века появилась первая работа, в которой экспериментально (на кроликах) было показано, что добавка холестерина к пище вызывает симптомы, напоминающие атеросклероз.
В дальнейшем, несмотря на очень большую условность модели (в нормальном питании кроликов продукты, содержащие холестерин, обычно отсутствуют) был сделан вывод о связи заболевания атеросклерозом с повышенным уровнем потребления и накопления холестерина в организме. Эта гипотеза была подтверждена в последующих работах с животными, в чей естественный рацион входит холестерин (крысы, обезьяны).
При наблюдении за большими популяциями людей, страдающих сердечно-сосудистыми заболеваниями также было установлено, что существует определённая взаимосвязь между повышенным уровнем холестерина и увеличением вероятности сердечно-сосудистых заболеваний.
Применение гиполипидемических средств давало некоторый статистически значимый эффект, однако для лиц старше 70 лет эта закономерность становится менее очевидной.
Основные группы веществ, регулирующих обмен липидов [ править | править код ]
C10AA ингибиторы 3-гидрокси-3-метилглутарил-СоА редуктазы («статины») [ править | править код ]
Группа лекарственных препаратов (часто их объединяют под условным названием «статины») предназначеных для снижения концентрации «холестерина» (ЛПНП, ЛПСП, ЛПВП) в крови человека. Первый в мире статин создан американской компанией Merck Sharp and Dohme.
- После триумфального старта первых статинов были получены данные о некоторых важных побочных эффектах и противопоказаниях. Основными побочными эффектами являются бессимптомное повышение уровня трансаминаз [1] , миопатия, боли в животе, запоры [2] . Первое поколение статинов (ловастатин, симвастатин, правастатин) сменилось вторым (флувастатин), третьим (аторвастатин, церивастатин) и четвёртым (питавастатин, розувастатин), и сегодня подход к их применению стал намного более взвешенным. В настоящее время деление статинов по поколениям считается безосновательным, так как не было показано существенное различие в их эффективности.
Так, в статье «Статины с позиции доказательной медицины» [3] Ю. С. Рудык, указывая на определённые успехи в использовании статинов, напоминает «становится очевидно, что применение статинов не решает полностью проблемы предупреждения осложнений ИБС и её эквивалентов… „… наступает момент, когда возможности статинов становятся исчерпанными, и дальнейшее совершенствование препаратов, увеличение их активности не приведет к существенному улучшению клинических результатов. Поэтому усилия врачей обращены на другие пути воздействия на атеросклеротический процесс, и дальнейший прогресс будет, вероятно, связан уже не со статинами“». «В странах благополучной Западной Европы 70 % пациентов получают статины для снижения уровня ХС, однако целевые значения достигают только у 53 % больных.» (там же).
- Клиническая эффективность статинов считается доказанной для лиц среднего возраста.
- Кроме того, известно, что уровень «холестерина» (ЛПНП, ЛПСП, ЛПВП) в крови далеко не всегда коррелирует с развитием атеросклероза, и, как следствие — с ростом риска смерти от сердечно-сосудистых заболеваний.

Кроме того, исследователи из Лондонского университета выяснили, что статины не только снижают уровень холестерина, но и улучшают структуру и функцию сердца. Люди, принимающие данные лекарства, реже имеют увеличенное сердце — признак стресса и мышечной слабости. По сравнению с обычными людьми, у пациентов, принимавших лекарства, левый желудочек сердца имел на 2,4% меньше мышечной массы. Уменьшены были также объемы левого и правого желудочков. [5]
Лекарственные препараты, которые не рекомендовано комбинировать со статинами из-за риска развития миозитов и рабдомиолиза [6]
- фибраты (риск рабдомиолиза и гепатотоксичности, возможно сочетание с флуваставтином)
- никотиновая кислота и её производные (риск гепатотоксичности)
- макролидные антибиотики (эритромицин, кларитромицин в особенности)
- циклоспорин
- азольные противогрибковые средства
- верапамил
- амиодарон
- ингибиторы протеазы ВИЧ
Состояния, которые увеличивают риск развития миозитов и рабдомиолиза при применении статинов:
В этих случаях больные должны находится под более тщательным наблюдением врача с контролем ферментов (АЛТ, АСТ, ГГТП, КФК) не реже 2 раз в месяц. Фармакокинетические исследования показали, что больные, принадлежащие к монголоидной расе, более чувствительны к действию статинов, поэтому у таких пациентов следует использовать меньшие дозировки ( C10AB Фибраты [ править | править код ]
Показания к назначению фибратов: IIб фенотип с высоким уровнем ТГ, ремнантный III и IV,V типы с высоким риском панкреатита, сниженный ХС ЛВП. При применении фибратов необходим УЗ контроль за состоянием желчного пузыря (увеличивают литогенность желчи).

Лекарственные препараты, которые не рекомендовано применять с препаратами никотиновой кислоты из-за риска развития побочных эффектов:
- статины (риск гепатотоксичности)
- фибраты (риск гепатотоксичности и рабдомиолиза)
Необходим контроль АЛТ, АСТ, ГГТП.
- У больных с сахарным диабетом и подагрой возможно обострение основного заболевания, у данной категории больных необходимо избегать назначения любых форм никотиновой кислоты.
C10AX Прочие гиполипидемические препараты [ править | править код ]
В ходе многочисленных клинических исследований с применением поликозанола и статинов были получены данные о том, что эффективность поликозанола и статинов для снижения ЛПНП примерно одинакова, при этом поликозанол значительно эффективнее препаратов из группы статинов повышает ЛПВП. В отличие от статинов, поликозанол не оказывает побочных эффектов на другие органы и системы организма. [7] [8] [9] [10] Также доказано, что поликозанол способствует снижению до нормы уровня аланин-аминотрансферазы (АлАТ) и гамма-глутамилтранспептидазы (ГГТ) [11]
В зависимости от наличия других факторов риска, атеросклероза и имевших место клинических его проявлений (в прошлом или настоящем), всех взрослых людей медики разделили на 3 группы риска, каждая из которых имеет свой допустимый уровень холестерола.
Роль ХС в человеческой смерти проста: он заполняет сосуды вместе с жирами, образуя на стенках атеросклеротические бляшки. Начало трагедии наступает тогда, когда прикрепляются к ней тромбоциты, перекрывающие сосуд.
Кровоснабжение прекращается, если бляшка в сосуде сердца, развивается инфаркт миокарда, если в головном мозге – инсульт, если в конечностях – гангрена ноги.
Долгие годы в арсенале медиков единственным методом борьбы с повышенным холестеролом был контроль его уровня с последующим назначением статинов – класса препаратов для снижения холестерина и лечения атеросклероза. Новые исследования в медицине заставили специалистов по-другому взглянуть на эту проблему.
Статины – что это?
Эти препараты занимают первое место по количеству больных, которые их принимают. Механизм воздействия понижающих липиды медикаментов основан на угнетении ферментов со сложным названием «редуктаза ГМГ-КоА», провоцирующих выработку в печени нового холестерола.
Статины восстанавливают поврежденные сосуды, когда атеросклероз еще не выявлен, но в стенках уже накапливается «плохой» ХС. Улучшают лекарства и реологические возможности крови: понижают вязкость, предупреждают появление тромбов.
По эффективности лидируют препараты от холестерина повышенного нового поколения, изготовленные на основе аторвастатина, церивастатина, розувастатина и питавастатина.
Статины не просто сокращают концентрацию вредного холестерола, но и увеличивают содержание полезного. Результат от применения лекарств этой группы можно увидеть уже через месяц после регулярного применения. Прописывают статины 1 раз в день, на ночь, допускается сочетание в одной таблетке и кардиологических средств.
Самолечение статинами недопустимо, так как рекомендации врача основываются на результатах биохимических исследований крови, в частности, на показаниях ЛПНП. Когда этот параметр не превышает 6,5 ммоль/л, его снижают коррекцией диеты и образа жизни. Если этих мер недостаточно, через полгода доктор назначает статины.
Непосвященному потребителю непросто разобраться: розувастатин и аторвастатин – в чем разница между этими и другими аналогичными препаратами-ингибиторами фермента, синтезирующего ХС? Розувастатин – препарат нового поколения, выгодно отличающийся от своих предшественников.
При эквивалентных с аторвастатином дозах он оказывает более выраженное воздействие. Важным аргументом будет и его более низкая токсичность.

Лекарства, продлевающие жизнь
Если немедикаментозная терапия оказывалась неэффективной, традиционно главным показанием для назначения ингибиторов была гиперхолестеринемия (в том числе и высокое содержание ХС генетической природы).
Сегодня статины назначают даже при нормальных показателях холестерина:
- Пациентам с ИБС;
- После инфаркта миокарда;
- После любой операции на коронарных артериях (стентирование, шунтирование);
- Если больной перенес инсульт;
- При сахарном диабете с высокими показателями ЛПНП.
Действие статинов гораздо шире, чем простая нормализация уровня холестерина, глобально – это лекарства, продлевающие жизнь. Решающий фактор при назначении статинов – это активно прогрессирующий атеросклероз. Все перечисленные патологии, а также наследственная предрасположенность, обеспечивают повышенный риск повреждения сосудов.
Этот класс медикаментов противопоказан при гепатите, циррозе и других патологиях печени. Не рекомендуют статины при беременности и в период грудного вскармливания. Не стоит принимать их и женщинам детородного возраста, если они не защищены надежными противозачаточными средствами. Не назначают статины, если выявлены аллергические реакции.
Статины не оказывают негативного влияния на другие процессы – обмен белков, углеводов, пуринов, поэтому их смело могут использовать диабетики, больные с подагрой и другими болезнями.
Побочные последствия
Подобные медикаменты при производстве подвергаются самому строгому контролю на предмет побочных явлений. Розувастатин, к примеру, изучали в течение двух лет, аторвасатин, ловастатин и симвастатин – в течение 3-5 лет. Кроме убедительной статистики по профилактике инфарктов, есть и другие плюсы.
Риск появления нежелательных последствий при продолжительном лечении статинами не превышает 1 %. Среди таких эффектов:
- Расстройство сна;
- Ухудшение слуха;
- Слабость и боли мышц и суставов;
- Разрушение мышц;
- Изменение восприятия вкуса;
- Тахикардия;
- Перепады АД;
- Снижение концентрации тромбоцитов;
- Кровотечения из носа;
- Диспептические расстройства;
- Нарушения ритма дефекации и мочеиспускания;
- Снижение сексуальной активности;
- Усиленное потоотделение;
- Аллергия.
Более 1% пациентов могут испытывать головокружения, боли в груди, кашель, отечность, высокую чувствительность к активному солнечному излучению, раздражения кожи (от покраснения до экземы).

Совместимость с другими медикаментами
ВОЗ и Американская Ассоциация Сердца рекомендуют статины как обязательный компонент при лечении ИБС, если риск возникновения осложнений достаточно высокий. Назначение этой категории больных только средств, понижающих ХС, недостаточно.
Стандартная терапия включает:
- в-блокаторы (типа бисопролола, атенолола, метопролола);
- Антиагреганты (в виде аспирина, аспикора, тромбо асс);
- Ингибиторы АПФ (периндоприл, квадриприл, эналаприл);
- Статины.
Многочисленные исследования подтверждают безопасность применения этих медикаментов в комплексе.
В некоторых случаях сочетание разных лекарственных средств в одной таблетке (например, правастатин + аспирин) понижает опасность возникновения инфаркта (всего лишь 7,6%) в сравнении с раздельным приемом этих лекарств (9% для правастатина и 11% — для аспирина).
Традиционно статины прописывали для приема на ночь, отдельно от остальных видов медикаментов. Сегодня фармацевты предлагают в одной таблетке комбинацию нескольких лекарств, такой вариант медики находят более предпочтительным. Среди таких препаратов – дуплекор, кадуэт, сочетающие в одной таблетке аторвастатин и амлодипин.
Проходит испытание и новое средство комплексного воздействия – Полипилл.
Если показатели ХС превышают 7.4 ммоль/л, статины сочетают с фибратами (альтернативная группа понижающих ХС лекарств). Какие статины самые эффективные и безопасные в конкретном случае, решает врач, анализирующий все возможные риски.
Недопустимо принимать статины, запивая таблетки грейпфрутовым соком, так как в его составе есть компоненты, угнетающие усвоение статинов. Повышение их содержания в крови опасно накоплением токсинов.
Несовместимо лечение препаратами этой группы с алкогольными напитками и некоторыми антибиотиками: такими как кларитромицин и эритромицин, создающими дополнительную нагрузку на печень.
Другие антибиотики вполне совместимы с препаратами, понижающими ХС. Состояние печени надо контролировать каждые 3 месяца, уточняя в анализах крови показатель печеночных ферментов.
Польза и вред статинов
Изучая рецепты, каждый здравомыслящий пациент задумывается об эффективности препаратов: насколько польза от статинов будет превышать возможный вред, о котором так много говорят? Развеять сомнения поможет информация о новых медикаментах с минимумом нежелательных последствий.
В пользу их эффективности говорят такие преимущества:
- Уменьшение процента смертей от сердечных болезней на 40% на протяжении 5 лет.
- Сокращение вероятности развития инфаркта и инсульта на 30%.
- Снижение показателей ХС на 45-55% (при регулярном и продолжительном употреблении). Для анализа динамики необходимо ежемесячно проверять кровь на уровень холестерола.
- Употребление терапевтической дозы статинов последнего поколения не дает токсических эффектов. Долгое время считалось, что статины способны увеличить риск развития рака печени, сахарного диабета 2 типа, катаракты, слабоумия. Современные исследования опровергли это заблуждение и доказали, что подобные последствия возникают по другим причинам. С 1996 года в Дании проводятся наблюдения за диабетиками. Вероятность возникновения осложнений диабета типа ретинопатии, полинейропатии сократилась на 40 и 34%.
- Большой выбор аналогичных препаратов различной стоимости с общим активным компонентом. На тематических форумах часто спрашивают: симвастатин или аторвастатин – что лучше? Первый вариант – представитель естественных статинов, второй – современный синтетический. При всем различии в строении и путях метаболизма, препараты имеют сходное фармакологическое действие. Существенно различаются они по цене: симвостатин значительно дешевле аторвастатина.
Среди недостатков можно отметить высокую стоимость крестора, розукарда, лескола форте и других оригинальных статинов последнего поколения, для каждого названия перечисленных препаратов всегда можно подобрать дженерик с доступной ценой.

Способы коррекции уровня ХС
Если холестерол повышен незначительно и особого риска сердечной недостаточности нет, надо пытаться нормализовать уровень диетой. Любопытно, что некоторые продукты питания растительного происхождения содержат натуральные статины. Наиболее изучены в этом плане возможности чеснока и куркумы.
Кроме них, правильный рацион включает щадящую термическую обработку продуктов (тушение, приготовление на пару, в запеченном или в отварном виде). Исключаются жирные и жареные блюда, есть ограничения в количестве яиц, молочных и субпродуктов.
Холестерин жизненно необходим организму как строительный материал для клеток, поэтому важно не исключать, а всего лишь ограничивать потребление некоторых видов продуктов.
Полезны растительная клетчатка (овощи, крупы, бобовые) и полиненасыщенные жирные кислоты Щ-3 (красная рыба, рыбный жир), нормализующие холестериновый обмен.
Если перечисленные способы не дают желаемого результата, назначаются гиполипидемические средства.
В заключение стоит подчеркнуть, что при всех вполне понятных страхах пациентов перед приемом статинов – препаратов для снижения холестерина – и распространенном мнении о вредных последствиях такого лечения, их назначение абсолютно оправдано при серьезном атеросклерозе с тяжелыми последствиями, так как эти лекарства вполне реально продлевают жизнь и улучшают ее качество.
Конечно, таблетку выпить проще, но при слегка повышенном холестерине без малейших признаков поражения сосудов все же лучше просто придерживаться здорового образа жизни и периодически контролировать показатели ХС.
