Пролежень (гангрена декубитальная) — некроз мягких тканей (кожи с подкожной клетчаткой, слизистой оболочки, стенки полого органа или кровеносного сосуда и др.), возникающий вследствие ишемии, вызванной продолжительным непрерывным механическим давлением. Локализация — выступающие участки костей: локти, ягодицы, пятки, лодыжки, крестец, позвоночник. Частота — 43:100 000; 9% госпитализированных больных: у 66% пожилых с переломами бедра, у 33% пациентов отделений интенсивной терапии, у 60% пациентов с тетраплегией; у больных, находящихся на постельном режиме, лечащихся на дому, — от 2,6 до 24%.
Код по международной классификации болезней МКБ-10:
- L89 Декубитальная язва
Преобладающий возраст — пожилой (60–70% пациентов).
Классификация • Экзогенный •• Возникает только под влиянием давления •• При отсутствии способствующих его развитию нарушений общего состояния организма (например, возникает при неудачно наложенной гипсовой повязке) • Эндогенный (невротический) развивается на фоне грубых нарушений нервной трофики тканей, чаще при тяжёлых повреждениях спинного мозга.
Этиология • Давление на места наиболее частой локализации, оказанное массой тела или конечностей, в течение 2 ч вызывает необратимую ишемию тканей и некроз • Силы трения при передвижении пациента по простыне • Влажность вследствие недержания мочи или потоотделения увеличивает трение между двумя поверхностями.
Факторы риска • Неподвижность (например, тетраплегия) • Недоедание и низкая масса тела • Гипоальбуминемия • Недержание кала • Недержание мочи • Переломы костей (особенно бедренной) • Дефицит витамина C • Низкое диастолическое АД • Связанные с возрастом изменения кожи: повышение порога болевой чувствительности, истончение эпидермиса, атрофия сосудов кожи, снижение иммунитета • Инфекционные болезни • Анемия • Заболевания периферических сосудов • СД • Злокачественные новообразования • Деменция • Мозговые сосудистые заболевания (инсульт) • Сухая кожа (сниженная влажность и холодность) • Отёки.
Симптомы (признаки)
Клиническая картина
• Стадия I: эритема без побледнения, поражённая область тёплая на ощупь, болезненна.
• Стадия II: •• Нарушение целостности кожи, ограниченное дермой •• Экскориации •• Наличие пузырей с отделяемым •• Эритема выражена более ярко, повышение температуры поражённой области •• Отёк.
• Стадия III: формирование язвы с отделяемым (язва распространяется не глубже дермы), заживление под струпом.
• Стадия IV: •• Язвы распространяются на мышцу или кость •• Некроз прилегающих тканей (видимое повреждение не отражает истинной площади некроза) •• Грануляции и эпителизация по краям раны.
Лабораторные исследования • Посев отделяемого раны • ОАК при наличии лихорадки • При лейкоцитозе — ОАМ, чтобы идентифицировать причинные факторы • Если вышеперечисленные исследования положительны — посев крови и мочи.
Дифференциальная диагностика • Лимфостаз или ишемические язвы • Васкулиты • Злокачественные новообразования • Лучевое повреждение • Гангренозная пиодермия.
Лечение
ЛЕЧЕНИЕ
Диета высококалорийная с высоким содержанием белков, добавление в рацион сульфата цинка, витаминов А и С, железа.
Физическая активность — пассивные движения в максимально достижимом для пациента диапазоне или активные упражнения, если они возможны.

Лекарственная терапия • Поддерживающая терапия: аскорбиновая кислота по 500 мг 2 р/сут • Противогрибковые: миконазол, клотримазол • Антибактериальные средства: сульфадиазин серебра, сульфадиазин натрия, клиндамицин, гентамицин, хлорамфеникол • Ферментативные очищающие средства: коллагеназа, трипсин, плазмин дезоксирибонуклеаза, папаин, ируксол • Полиуретановые пленки • Абсорбирующие гидроколлоидные перевязки.
Профилактика • Ранняя идентификация индивидуумов из группы риска • Оценка состояния кожи у пациентов группы риска • Устранение факторов риска • Квалифицированный уход •• Облегчение давления на кожу: тщательное разглаживание простыней, частое перестилание постели (постельное бельё необходимо держать сухим), покрывала на матрацы, подушки, специальные матрацы и кровати, частое изменение положения тела пациента, обработка кожи камфорным спиртом •• Лечение недержания — использование прокладок, поглощающих избыток влаги •• Очищение кожи сразу после повреждения • Дополнительное введение белков и калорий, коррекция диеты с помощью витамина C и цинка.
Осложнения • Гангрена • Панникулит • Остеомиелит • Сепсис
Течение и прогноз • Средний срок госпитализации при лечения пролежней — 46 дней • При хорошем медицинском обслуживании состояние пациента с пролежнями в 79% улучшается, в 40% они полностью излечиваются • Риск летального исхода у пожилых неподвижных пациентов, несмотря на заживление язв, увеличивается в 4 раза и в 6 раз при наличии незаживающей язвы.
МКБ-10. L89 Декубитальная язва
Пролежни – это дегенеративный язвенный процесс в мягких тканях, сопровождающийся их постепенным омертвением. Пролежни (МКБ 10 код – L89) возникают у пациентов с ограниченной двигательной активностью на фоне длительного пребывания в статичной позе лежа на спине, животе или сидя в инвалидном кресле. Наиболее подвержены образованию повреждений на коже лежачие пациенты пожилого возраста с нарушениями периферического кровообращения.
Диагностика пролежней в Юсуповской производится в несколько этапов, основываясь на данных по Международной Классификации болезней (пролежни – код по МКБ 10 — L89) и анамнезе пациента. Причины развития пролежней
На фоне нарушенного кровообращения и циркуляции лимфы в верхних слоях кожи развивается застой, который провоцирует возникновение некроза. Процесс омертвения мягких тканей может развиться быстрее на фоне следующих заболеваний или особенностей организма:
- хронические заболевания дыхательной системы, сахарный диабет, нарушение работы сердечно-сосудистой системы и нервной трофики тканей при серьезных травмах спинного мозга;
- пожилой возраст. У пожилых людей способность кожных покровов к регенерации существенно снижается, происходит замедление обменных процессов. Кожа становится тоньше и хуже выполняет барьерную функцию между внешними раздражителями и костями. Это существенно увеличивает возможность возникновения пролежневых язв при условии постоянного нахождения в статичном положении;
- тучность человека или напротив, слишком маленькая масса тела. Большой вес способствует еще большему давлению тела на подлежащую поверхность, где находится человек, а низкий процент подкожной жировой клетчатки не обеспечивает достаточную функцию защиты мышц, сухожилий и костей от источников давления извне;
- малый процент потребления белковой пищи. Белки участвуют во всех клеточных обменных процессах в организме человека. У лежачих пациентов в случае их недостачи кожа теряет эластичность, клетки не возобновляются как прежде и возникает некроз тканей;
- трение. Трение, например, об простыни возникает на фоне постоянных передвижений пациента другими людьми. Механические повреждения способствуют образованию повреждений на коже, включая и пролежневые язвы;
- высокая влажность тела. Влажная среда является благоприятной для появления некроза тканей, а также присоединения побочных инфекций. Наиболее часто влажность появляется на фоне повышенной потливости или недержания мочи и кала;
- снижение чувствительности кожных покровов. Когда пациент не чувствует определённый участок тела, диагностировать болезнь гораздо тяжелее, поскольку субъективные ощущения играют важную роль в оценке пролежня;
- курение.
Категории пролежней – пролежни МКБ
В современной медицине пролежни классифицируются не по степеням, а по категориям. Существует четыре категории пролежней:
- 1 категория. На месте сдавливания тканей образуется локальное покраснение (венозная эритема) розового или синюшного цвета, не проходящая даже после прекращения сдавливания. Повышается температура тела, возможны головные боли или головокружения. Чувствительность кожных покровов обострена;
- 2 категория. Целостность кожных покровов нарушается, возникает неглубокая рана, затрагивающая эпидермис. Ложе раны имеет желтоватый или белый цвет;
- 3 категория. Язва достигает слоев дермы, мышц и сухожилий, задевается подкожная жировая клетчатка. Края раны приобретают серый цвет, на ложе пролежня присутствует спрут или некротические массы;
- 4 стадия. Кожный покров обнажается до капсульно-связывающего аппарата и костей. Рана имеет спрут или нагноение в случае присоединения вторичной инфекции.
Чаще всего пролежни формируются на местах непосредственного контакта костных или мышечных выступов с твердой поверхностью. Если пациент лежит на спине, этими местами являются: лопатки, пятки, затылок, локти, ягодицы, крестец (пролежень крестца код по МКБ — L89) и область икр. Если пациент лежит на спине, есть риск появления пролежней в районе лобка, коленей, подвздошной кости и подбородка.
Профилактика пролежней
Несмотря на возможное наличие внутренних заболеваний, способствующих появлению пролежней, решающим фактором в их развитии становится некачественный уход за пациентом. Регулярная профилактика пролежней, которая может осуществляться в стенах стационара Юсуповской больницы, обеспечивает лежачему больному безопасное пребывание в клинике. Противопролежневый матрас с помощью специального механизма обеспечивает периодический массаж всех зон тела, не позволяя образоваться застою или пережатию кровеносных капилляров. Подкладной круг используют в местах соприкосновения кожи с поверхностью. Полость внутри круга позволяет ране не контактировать с кроватью, не нарушая микроциркуляцию и позволяя регенеративному процессу проходить быстрее. Надувные подушки используются для сидячих пациентов;
- качественная гигиена пациента, проведение обеззараживающих и уходовых манипуляций. Обеззараживание пролежней производится мягкими антисептическими препаратами, назначаемыми лечащим врачом, после чего накладывается специальная повязка с ранозаживляющими мазями. Личная гигиена лежачего больного состоит из регулярной смены нательного и постельного белья, а также памперсов, если у человека имеются нарушения мочевыводящей функции или недержание кала. Мыть пациента следует ежедневно под теплой проточной водой с использованием мягких моющих средств.

Причины возникновения [ править | править код ]
- Недостаточный уход за больным.
- Слишком большой или слишком малый вес пациента.
- Сухость кожи.
- Недержание мочи или кала.
- Заболевания, приводящие к нарушению трофики (питания) тканей.
- Анемия.
- Ограниченная подвижность.
- Повышение или понижение температуры тела.
- Недостаточное белковое питание.
Стадии пролежней [ править | править код ]
Степень I: кожный покров не нарушен. Устойчивая гиперемия, не проходящая после прекращения давления.
Степень II: поверхностное (неглубокое) нарушение целостности кожных покровов с распространением на подкожную клетчатку. Стойкая гиперемия. Отслойка эпидермиса.
Степень III: разрушение кожного покрова вплоть до мышечного слоя с проникновением в мышцу. Пролежень выглядит как рана. Могут быть жидкие выделения.
Степень IV: поражение всех мягких тканей. Наличие полости, обнажающей нижележащие ткани (сухожилия, вплоть до кости).
При пролежнях III-IV степени главный метод лечения — хирургический.
Места образования [ править | править код ]
Пролежни образуются в большей степени в области крестца, лопаток, пяток, коленей, ребер, пальцев ног, больших вертелов бедренной кости, стоп, седалищной кости, гребней подвздошной кости и локтевых суставов. В мировой практике известны также случаи локализации пролежней на пальцах рук, а также на голове и ушах; поражаются кожа (поверхностный пролежень) и подкожная клетчатка с мышцами (глубокий пролежень, который опасен образованием инфицированной раны). Пролежень на коже может возникнуть и от давления гипсовой повязки при переломах или на слизистой оболочке рта — от давления зубного протеза и т. д. Основные причины локализации пролежней — это давление и время. Если наружное давление на протяжении длительного времени (более двух часов) будет выше, чем давление внутри капилляров, то образование пролежня практически неизбежно.
Лечение [ править | править код ]
Основные средства лечения пролежней [3] :
- Антибактериальные препараты и фунгицидные средства;
- Некролитические препараты (коллагеназа, дезокси-рибонуклеаза, трипсин, химотрипсин, террилитин);
- Дегидратирующие — гиперосмолярные препараты;
- Ангиопротекторы — средства, улучшающие микроциркуляцию (пирикарбат, трибенозид);
- Противовоспалительные средства (дексаметазон, гидрокортизон, преднизолон);
- Стимуляторы репаративных процессов (Стелланин, метилурацил, винилин, мазь каланхоэ);
- Современные серебросодержащие препараты (Арговит, Аргокрем, Аргогель, Аргосульфан, Дермазин).
Профилактика [ править | править код ]
Тщательный уход за кожей (протирание), регулярная смена белья и перемена положения больного в постели обязательно каждые 2 часа, применение специального массирующего (водного, пневматического и т. д.) матраца и других специальных приспособлений.
Особое место в лечении должно занимать питание больного. В рационе должна преобладать богатая белками пища. В сутки должно быть выпито не менее 1,5 литров жидкости.
Профилактика появления пролежней начинается уже в первые дни вынужденной обездвиженности больного. Хорошо, если в медицинском учреждении пациенту предоставлена специальная многофункциональная кровать для лежачих больных. Механизм кровати позволяет приподнять голову человека или ноги, придать ему полусидячее положение. Таким образом будет меняться давление на различные части тела, будет облегчен уход за больным. Необходимо строго следить за своевременностью гигиенических процедур, сухостью кожных покровов, чистотой постельного белья, отсутствием складок и крошек на белье.
Клинические признаки пролежней у животных [ править | править код ]
Внешне пролежни характеризуются наличием на выступающих боковых поверхностях тела с одной или двух сторон очагов омертвевшей кожи различной величины, округлой или овальной формы. Кожа утончена, плотная, буро-чёрного цвета, волосы взъерошены или отсутствуют. Окружающие ткани в свежих случаях и при осложнении инфекцией отёчны, болезненны. Позднее, по мере отторжения, тонкий край мёртвой кожи завёртывается наружу. При глубоких поражениях из-под него выделяется гной. [4]
Лечение и профилактика у животных [ править | править код ]
Больные животные, вынужденные больше лежать, должны быть обеспечены обильной подстилкой из соломы, торфа, опилок или их комбинаций. При этом тяжелобольных следует регулярно, через 2-3 часа, переворачивать на другую сторону. Для улучшения кровообращения сдавливаемые участки протирают камфорным спиртом. При появлении пролежней кожу пораженного участка обрабатывают 1-2 раза в день 2-3%-м спиртовым раствором пиоктанина, бриллиантовой зелени или мазями-цинковой, йодоформтаниновой. [4]
