Показания

Инструкция по применению и дозировка
При приеме внутрь взрослым — по 25 мг 3-4 раза в сутки, детям в зависимости от возраста — по 6.25-12.5 мг 2-3 раза в сутки. Максимальная доза для взрослых составляет 150 мг в сутки.
Внутривенно или внутримышечно.
Взрослым назначают 20-40 мг в сутки (содержимое 1-2 ампул).
Лечение у детей начинают с дозы 5 мг (0.25 мл). В дальнейшем дозу повышают в зависимости от возраста ребенка.
Детям в возрасте 1-12 месяцев — 5 мг (0.25 мл).
Детям в возрасте 1-6 лет — 10 мг (0.5 мл).
Детям в возрасте 7-18 лет — 10-20 мг (0.5-1 мл).
Суточная доза не должна превышать 2 мг/кг массы тела.
При тяжелом течении аллергии лечение следует начинать с инъекционной формы хлоропирамина. Сначала его вводят в/в медленно, затем переходят на в/м введение и в заключительной фазе лечения переходят на прием таблеток внутрь. Сроки лечения устанавливают индивидуально в зависимости от клинической симптоматики и состояния больного.
Пожилые, истощенные больные: применение препарата хлоропирамин требует особой осторожности, т.к. у этих больных антигистаминные препараты чаще вызывают побочные эффекты (головокружение, сонливость).
Пациенты с нарушением функции печени: может потребоваться снижение дозы в связи со снижением метаболизма активного компонента препарата при заболеваниях печени.

При приеме на ночь хлоропирамин может усиливать симптомы рефлюкс-эзофагита.
Антигистаминные средства могут искажать проявление реакции при постановке кожных аллергологических проб, поэтому за несколько дней до планируемого проведения теста применение препарата следует прекратить.
При сочетании с ототоксичными препаратами хлоропирамин может маскировать ранние проявления ототоксичности.
Во время лечения следует исключить употребление алкоголя.
Влияние на способность к вождению автотранспорта и управлению механизмами
В начальном, индивидуально определяемом периоде применения хлоропирамина не разрешается вождение транспортных средств и занятия другими потенциально опасными видами деятельности, требующими быстроты психомоторных реакций. В процессе дальнейшего лечения степень ограничений определяют в зависимости от переносимости лечения.
Лекарственное взаимодействие
При одновременном применении хлоропирамин потенцирует эффекты средств для наркоза, снотворных средств, транквилизаторов, анальгетиков, ингибиторов МАО, трициклических антидепрессантов, атропина и симпатолитиков (такая комбинация требует осторожности).
При одновременном применении хлоропирамина и кофеина или фенамина отмечается уменьшение или устранение угнетающего действия хлоропирамина на ЦНС.

Фармакодинамика
Хлоропирамин является блокатором Н1-гистаминовых рецепторов. По химическому строению относится к производным этилендиамина. Хлоропирамин селективно блокирует Н1-гистаминовые рецепторы и уменьшает проницаемость капилляров. Предупреждает развитие и облегчает течение аллергических реакций. Оказывает седативное, антигистаминное, м-холиноблокирующее и выраженное противозудное действие. Обладает умеренной спазмолитической активностью и противорвотным действием.
Фармакокинетика
Равномерно распределяется в организме, включая центральную нервную систему. Проникает через гематоэнцефалический барьер. Связь хлоропирамина с белками плазмы крови составляет 7,9%. Пик связывания отмечен при pH 6,8-7,4. Терапевтическая концентрация сохраняется 4-6 ч. Интенсивно метаболизируется в печени.
Выводится преимущественно через почки с мочой в виде метаболитов. Выведение хлоропирамина у детей может происходить быстрее, чем у взрослых.

Со стороны центральной нервной системы: вялость, слабость, сонливость, головокружение, головная боль, эйфория, ощущение усталости, раздражительность, тремор, судороги, нарушение координации движений, нарушение зрения; у детей возможно некоторое стимулирующее действие на центральную нервную систему, проявляющееся беспокойством, повышенной раздражимостью, бессонницей.
Со стороны органов пищеварения: сухость во рту, тошнота, рвота, гастралгия, диарея/запор, потеря или повышение аппетита.
Со стороны мочеиспускательной системы: затруднение мочеиспускания.
Со стороны сердечно-сосудистой системы: снижение артериального давления (чаще у пожилых пациентов), тахикардия, аритмия.
Со стороны системы кроветворения: очень редко – лейкопения, агранулоцитоз.
Со стороны органов зрения повышение внутриглазного давления.
Аллергические реакции к препарату.
Прочие: фотосенсибилизация, мышечная слабость.
Передозировка
Симптомы: у детей – возбуждение, тревожность, галлюцинации, атетоз, атаксия, судороги, мидриаз, неподвижность зрачков, гиперемия кожи лица, гипертермия; затем – коллапс, кома. У взрослых – угнетение (заторможенность, депрессия) или возбуждение центральной нервной системы (психомоторное возбуждение), очень редко – галлюцинации, расстройство координации, расширение зрачков и отсутствие их реакции на свет, сухость во рту, гипертермия и гиперемия кожных покровов, судороги, в тяжелых случаях – коматозное состояние.
Лечение: симптоматическая терапия (включая назначение противоэпилептических средств, кофеина, фенамина), реанимационные мероприятия, искусственная вентиляция легких – по показаниям. Специфического антидота нет.
Взаимодействие с другими лекарственными средствами
Усиливает действие лекарственных средств для общей анестезии, снотворных, седативных лекарственных средств, транквилизаторов, м-холиноблокаторов, опиоидных анальгетиков, местных анестетиков.
Трициклические антидепрессанты и ингибиторы моноаминоксидазы усиливают м-холиноблокирующее и угнетающее действие на центральную нервную систему.
Кофеин и фенамин уменьшают угнетающее действие на центральную нервную систему.
Одновременное применение с этанолом не рекомендуется (риск тяжелого угнетения ЦНС).
Особые указания
Во время лечения недопустимо употребление алкогольных напитков. Прием препарата на ночь может усилить симптомы рефлюкс-эзофагита. Если во время длительного приема препарата наблюдается беспричинное повышение температуры тела, ларингит, бледность кожных покровов, желтуха, образование язв слизистой оболочки полости рта, появление гематом, необычные и трудноостанавливаемые кровотечения, необходимо обратиться к врачу, так как пролонгированная терапия антигистаминными препаратами может привести к нарушениям гемопоэза. Антигистаминные средства могут искажать проявление реакции при постановке кожных аллергологических проб, поэтому за несколько дней до планируемого проведения теста применение препарата следует прекратить. Больным, применяющим хлоропирамин, рекомендуется воздерживаться от вождения транспортных средств, работы с механизмами, а также от других видов деятельности, требующих повышенной концентрации внимания и быстроты психомоторных реакций.

Распределение

Препарат также оказывает действие на гладкую мускулатуру, проницаемость капилляров и центральную нервную систему.Терапевтический эффект хлоропирамина продолжается минимум 3-6 ч.
Хлоропирамин предупреждает развитие и облегчает течение аллергических реакций.
Оказывает седативное и выраженное противозудное действие.
Обладает умеренной периферической антихолинергической и спазмолитической активностью и противорвотным действием.
Показания к применению Хлоропирамин

Применение Хлоропирамин (способ, дозировка)
Внутримышечно (в/м), внутривенно (в/в) (применяется только в острых тяжелых случаях под контролем врача!).
Взрослым и подросткам с 14 лет назначают в/м по 20-40 мг в сутки (содержимое 1-2 ампул).
- в возрасте от 1 до 12 месяцев — 5 мг (содержимое 1/4 ампулы) в/м;
- от 1 до 6 лет — 10 мг (содержимое 1/2 ампулы) в/м;
- от 6 до 14 лет — 10-20 мг (содержимое 1/2-1 ампулы) в/м.
Дозу можно осторожно повышать в зависимости от реакции пациента и наблюдаемых побочных эффектов.
Максимальная суточная доза не должна превышать 2 мг/кг массы тела.
При анафилактическом шоке или острой тяжелой аллергической реакции лечение рекомендуется начинать с осторожной медленной внутривенной инъекции, после чего продолжать внутримышечные инъекции или перейти на прием пероральных форм.
Сроки лечения устанавливаются индивидуально в зависимости от клинической симптоматики и состояния больного.

Побочные действия Хлоропирамин
Отсутствуют современные клинические данные, которые могли бы быть использованы для определения частоты побочных эффектов.
При применении Хлоропирамина могут возникнуть следующие побочные эффекты:
- седативный эффект, утомляемость, головокружение, атаксия, тремор, нервное возбуждение, судороги, головная боль, эйфория, энцефалопатия, нарушения остроты зрения;
- артериальная гипотензия, тахикардия, аритмия;
- потеря или повышение аппетита, неприятные ощущения или боль в эпигастральной области, сухость во рту, тошнота, рвота, диарея, запор;
- дизурия, задержка мочи;
- миопатия;
- повышение внутриглазного давления, приступ глаукомы;
- фотосенсибилизация.
Редко: лейкопения, агранулоцитоз, гемолитическая анемия, другие нарушения со стороны системы крови.
Противопоказания Хлоропирамин
- индивидуальная чувствительность к хлоропирамину или другим производным этилендиамина;
- острый приступ бронхиальной астмы;
- язвенная болезнь желудка в стадии обострения;
- закрытоугольная глаукома;

Лекарственные взаимодействия
Ингибиторы моноаминооксидазы (МАО), например, моклобемид или селегилин усиливают и удлиняют антихолинергические эффекты Хлоропирамина.
Следует соблюдать особую осторожность при одновременном применении Хлоропирамина с барбитуратами, седативными средствами, транквилизаторами, опиодными анальгетиками, трициклическими антидепрессантами, атропином и другими мускаринергическими парасимпатолитиками, поскольку они усиливают действие друг друга.
При сочетании с ототоксическими препаратами Хлоропирамин может маскировать ранние признаки ототоксичности.
Антигистаминные препараты могут подавлять реакцию кожи на аллергические тесты, поэтому прием Хлоропирамина должен быть прекращен за несколько дней до проведения кожных проб.
Не совместим с этанолом.
Особые указания
В связи с наличием антихолинергического и седативного эффектов следует с осторожностью назначать Хлоропирамин пациентам пожилого возраста, а также при нарушении функции печени, сердечно-сосудистой системы, задержке мочи.
Если препарат принимается поздно вечером, он может усилить симптомы гастроэзофагеального рефлюкса.
Побочное седативное действие антигистаминных препаратов на центральную нервную систему может усилиться под действием алкоголя, поэтому во время лечения Хлоропирамином следует воздержаться от употребления алкогольных напитков.
Длительное применение Хлоропирамина в редких случаях может вызывать нарушения со стороны системы кроветворения (лейкопению, агранулоцитоз, тромбоцитопению, гемолитическую анемию).
При возникновении лихорадки неясного происхождения, ларингита, изъязвления слизистой оболочки рта, бледности, желтухи, кровоизлияний, необычных или трудно останавливаемых кровотечений, при длительном введении препарата следует определять количество форменных элементов крови, а при обнаружении нарушений со стороны системы кроветворения прием препарата следует прекратить.
Особенности влияния лекарственного средства на способность управлять транспортным средством или потенциально опасными механизмами
В связи с возможным головокружением и сонливостью на фоне лечения препаратом Хлоропирамин возможно снижение способности к концентрации внимания и скорости психомоторных реакций.
Передозировка Хлоропирамин
Симптомы: галлюцинации, возбуждение, атаксия, нарушение координации, атетоз, судороги.
У маленьких детей преобладает возбуждение.
Кроме того, могут наблюдаться сухость во рту, застывшие расширенные зрачки, гиперемия кожи лица, синусовая тахикардия, задержка мочи, лихорадка.
Передозировка антигистаминных препаратов, в том числе Хлоропирамина, особенно у младенцев и детей, может привести к смерти.
У взрослых — лихорадка и гиперемия кожи лица наблюдаются непостоянно, после периода возбуждения следуют судороги и послесудорожная депрессия, с возможным развитием комы и кардио-респираторной недостаточности, которые могут привести к смерти пациента в течение 2-18 часов.
